Student AccommodationsAccommodation Form

We are happy to provide accommodations to our students with disabilities. This includes physical disabilities, learning disabilities, and functional limitations. In order to receive accommodations, students must fully complete our request form and provide supporting documentation at least 30 days prior to the first day of the class, regardless of when they registered for the class. Our office must have adequate time to confirm the student's disability and determine their eligibility before accommodations can be approved. No accommodations can be provided without proper documentation/confirmation of the disability from a qualified medical professional. This policy also applies to the utilization of service/therapy/comfort animals. If a student has paid their tuition fee, but cannot attend a class because accommodations could not be approved in time, we will transfer their tuition fee to a future class without penalty. The student is responsible for meeting all course requirements using only the specific accommodations that have been approved.
